1. You need to submit a few forms for your first order. Print out all the forms directly from the screen by clicking here. You can also enroll on-line by clicking here.
   
   • Complete and sign the Confidential Questionnaire including your payment information
   • Attach a copy of your prescription
   • Email, Fax or Mail everything to 120 West S.R. 434, Winter Springs, FL 32708
     
     
2. Visit one of our Service Centers. For locations and hours of operation call 800 967-4582. Our friendly staff will assist you free of charge and help you complete the needed forms right there. We will also quote you the cost of your prescription and process your order. If you call ahead and make an appointment, there will be no wait.

Your file will be treated as strictly confidential and no information will be made available to anyone other than licensed physicians and pharmacists.

Once we receive your completed forms and prescriptions, it takes 10 days to process your order by a Canadian physician and pharmacist, plus 10 to 12 business days for shipping.
